My fiancee's phone had this problem recently. I found this video very helpful and it worked. Except that I suggested she try some compressed air (the kind in cans for cleaning keyboards) She tried the q-tip and it fixed it for a minute or two, but the compressed air cleaned some more out and it has been working great since.
